Option A: Cancellation of Subscription during cooling off period (i.e. within 14 days of subscribing):

I hereby give notice that I cancel my contract for subscription for WineSpark membership, which I ordered on [ state date that you signed up for WineSpark membership ]. [ __ ] tick

(I note that if I started to avail of membership services prior to sending this cancellation notice, such as joining an online seminar or ordering wines only available to members, I will lose my right to cancel my subscription.)

Option B: Cancellation of Subscription at the end of monthly subscription period:

I now give at least 3 days’ notice to WineSpark that I wish to cancel my contract for subscription for WineSpark membership, at the end of the current month of my membership subscription period. [ __ ] tick

Option C: Cancellation of order for products during cooling off period (ie w/in 14 days of receipt): [ __ ] tick
